I'm not exactly sure, but I think that we assume that the ratio of concentration from reactant to intermediate or intermediate to product is constant (everything is relative to each other), not that the intermediate has a constant concentration.

I pretty sure that the phosphate that shoots of the ATP binds to something else, and the energy released to create that bond is greater than the energy needed to break the bond between ADP and P

A tip I had when memorizing these
Their roles kinda flip (acid from acceptor to doner, and vice versa for base), which make sense, as protons and electrons are the opposite (strictly charge speaking) of each other

I think you may have rounded a bit too much
I, like you got C and O as my lowest numbers as 3.28, and H as 7.88. However, when you divide 7.88/3.28, you get 2.39 which, when approximated to 2.4, you can multiply by 5 to get 5:5:12 ratio of C:O:H ratio, which conveniently is the right molar mass
