Prefixes such as di-, tri-, tetra-, etc, are ignored when alphabetizing substituents. However the prefixes cyclo-, iso- and neo- are part of the name. I was looking over this a few days ago and also saw the 4H2O2. It has to be an error in the textbook since throughout the example only H2O is used.
