Bimolecular Nucleophilic Substitution Reaction

How do you tell if the rate limiting step is a second order reaction, thus a bimolecular nucleophilic substitution reaction?

Do bimolecular nucleophilic substitution reactions occur only in one step?

If a reaction occurs via a SN2 mechanism, that by definition means it is a bimolecular nucleophilic substitution reaction that occurs in one step.
