Arrow direction versus equilibrium arrow

Are there circumstances where the arrow in the reaction is strictly in one direction or can the equilibrium arrow generally be used?

When the arrow points in one direction, it means that the reaction is irreversible. The reactant has fully changed into a new product.

How do we know when a reaction is irreversible?
