Can we always assume that the systems that we are studying are Reversable unless stated otherwise?

It would usually tell you. If the external pressure changes that is a good indication that it is reversible.

Is there any example in the course reader of an irreversible process, besides what is stated about it on page 33? I'd like to have a better understanding of it.
