When a system expands reversibly, does that only mean that change in entropy of the total system and surroundings is 0?

Yes, when a system expands reversibly the total entropy of a system and its surroundings is unchanged.

Yes because whatever entropy the system loses is gained by the surroundings, so the entropy change of the universe is zero.
