delta S at constant temperature

How would we know if delta S = delta H/q or if delta S = -delta H/q? Are there any conditions where we would make the enthalpy value negative instead?

I think it just depends on if you're trying to find the entropy of the surroundings or the system, and also if the enthalpy value given is for the system or the surroundings. If they are both for the system or both for the surrounding, then positive delta H/q, but if one is for the surrounding and one is for the system, then you would use negative delta H/q
