When do I add the OH- instead of H2O or H+?

You would use OH- when balancing a basic solution. I would refer to toolbox 6K.1 and example 6K.2 in the 7th edition for a better guideline on the use of OH-.

You had OH- when dealing with bases because they are going to result in the addition of more bases as they accept hydrogens.

It would depend on each question and if the reaction was taking place in an acidic solution or a basic solution. And the question would specify which reaction it is. If it's an acidic solution, you add H+ and water; if it's a basic solution, you add OH- and water.
