Balancing redox reactions

How will we know whether to add H+ or OH- to the reaction? Will it say the rxn is done under basic/acidic conditions?

Yes, although in some cases it won't matter in which type of solution the reaction occurs.

When it is necessary to balance hydrogens, H+ is added when the reaction is specified as acidic, and OH- and H2O are added when it is basic.
