For the cell Cu(s)/Cu2+(aq)//M2+(aq)/M(s) how do you know that the Cu2+ is the anode and M2+ is the cathode?

It is conventional to write the anode on the left and the cathode on the right. If it is otherwise, the problem will usually specify.

If CU2+/Cu is .34 volts then wouldn't the reverse reaction where copper loses electrons be -.34?
