How exactly do we use the slope of a reaction graph to determine the rate constant k?

This depends on what order of reaction you are plotting. If the reaction is zero order, then you plot [A]t vs. t and the slope should equal -k. For first order, you would plot ln[A]t vs. t and the slope is -k. 2nd order, 1/[A]t vs. t plots yield a slope of k itself.

because your integrated rate laws can be viewed in the form of y=mx+b and k is in the place of m, you regard k as your slope. depending on the order, k will be positive or negative.
