Will catalysts always be in the first step of a reaction mechanism as a reactant?

In general, yes, catalysts are there to begin with, whereas intermediates are formed during the reaction mechanism. Both are neither reactants nor products. I think that it is possible to have catalysts in different steps though, since a couple HW questions asked about the effect of adding catalysts are different steps.

Yes, if it is not one of the reactants of the first step, it would be an intermediate instead.
