I know the 4s orbital fills first before the 3d orbital because the 4s orbital is lower in energy, but are electrons always removed from the 4s orbital first as well?

Much like the way the orbital fills in a specific order, electrons also remove in a particular way Because 3d has a higher energy, the electron will be removed from 4s first.

Yes, electrons are removed first from 4s rather than 3d orbital. This is due to the fact that 4s always have lower ionization energy, while 3d has higher ionization energy. Thus, the electrons are always being removed from the 4s orbital easily compared to 3d, which contains filled shells.

And I think this becomes even more clear when you write the electron configuration of a given atom. Whenever, you write the electron configuration with an atom that has electrons in a "d-orbital" state, you write the d-orbital before you write the s-orbital. Thus, you remove the electron from the s-orbital before the d-orbital.

Is it because 4s is in a higher shell than 3d? Since 4s is in the shell further away from the nucleus than 3d, it would be removed first?

Yes, electrons with higher energy level are always removed first, and always in -f -p -d -s order
