Octet rule exceptions?

What are the exceptions to the octet rule?

In general, the first four elements along with boron's group are an exception and can have six valence electrons. Anything past, P in the periods can have more than an octet as they begin to occupy the d orbital.

Expanded octets are usually only possible for elements in period 3 or beyond.

It's usually possible in periods 3 and beyond because they have an empty d-orbital (3d etc.) that allows them to accept those extra electrons.

I think it's the first elements where they simply don't have enough electrons to fill an octet and anything after the third period where expanded octets can occur.
