How is the Lewis structure for this one correct if neither of them have full octects and the formal charge is a 4?

I’m guessing you are referring to part (a) of 3.59 ? because that is the only one without a full octet but this is because it's not possible to have a full octet on one of the elements since ClO is radical since there is an odd number of total valence electrons for ClO since Cl has 7 valence electrons and O has 6 so 13 in total. This Lewis structure is the closest you can get to satisfying the octet rule, except only putting 7 around Cl. I’m not sure where you got a formal charge of 4, there is a FC of +1 on Cl and -1 on O thus a total FC of 0.
