Do we have to memorize the molecular shapes and bond angles for the test or will be we given a chart?

You have to memorize them, don't rely on a table on the test. I believe you need to know bond angles, shape, and whether molecules are polar or not.

I think we will have to memorize the molecular shapes and bonds angles based off the Lewis structures.

You should know the bond angles and shape names.
