affect of multiple lone pairs on the central atom

I understand how having a single lone pair affects the molecular shape of a molecule, but how does having multiple lone pairs on the central atom affect the shape? Do the lone pairs go one side as one electron cloud, or are they one different regions around the central atom?

For multiple lone pairs, think of them as electron domains which do not contribute to bond formation. They will just stay there, but won't combine.
