If the molecule or atom has a negative or positive charge it is best to portray so, if it is neutral there is really no need.

If the formal charge on an atom is not zero then it is good to label it. You would then add up the formal charges in the molecule and label that outside brackets to depict the overal charge of the molecule.

I think you should always include the formal charge if it is not equal to 0. That is at least what sapling wants so I would assume it would be the same for an exam

It's better to include the formal charge of an atom/molecule especially if the formal charge isn't zero.

If the formal charge is not zero, it is in your favor to include the charge regardless if it is positive or negative.
