how do we determine the change in molar concentration?

Can you further explain your question? Do you mean in terms of change in molar concentration for equilibrium or for change in molar concentration in one molecule in general?

Like on the ICE chart, how do we know if it should be +x,-x, +2x, etc.?

First, determine if Q<K or if Q>K. For example, if there are little or no products to start with, then Q<K so the reaction will proceed forward. Then you know that the reactants will have negative changes and the products will have positive changes.

The coefficient of the change (x, 2x etc) is simply the coefficient of that species in the balanced reaction.
