This equation/relationship always hold true for reactions in water at 25 degrees celsius, but can be different at other temperatures.

We know that this specifically always will hold true for reactions at 25 C but not the same for other temperatures. We usually are given a temperature and I think we assume it as at 25 C if we are not given one.

Yes, this is true, but only for 25 degrees Celsius! It changes for other temperatures, which is why he emphasizes that its at 25 degree Celsius in lectures.

Yes, however it's not an issue when thinking about it because all problems in the textbook problems and achieve questions are written with the implication that the temperature is set at 25 degrees Celsius.
