H3O vs H2O

I'm confused on why some equations we've been seeing in class result in H3O+ being formed and others have H2O being formed. Is it because when an acid or base reacts with water it forms H3O+? And when an acid and vase react they produce H2O? Or am I totally off?

When an acid reacts with water, H3O+ is formed.
HA + H2O --> A + H3O+

When a base reacts with water, OH is formed.
B + H2O --> BH+ + OH

When an acid reacts with a base, H2O is sometimes formed.
HBr + KOH --> KBr + H2O

In the last example with HBr and KOH, what is the acid and what is the base?

HBr is the acid in this example and KOH is the base.

Also, since acids donate hydrogen ions, if the product has less hydrogen than the one in the reactant side, then it is easier to determine the acid.

When he spoke about H30+ being formed he was talking about putting an acid into water, whereas when H2O is formed he was talking about neutralization reactions where when you but an acid and base together they form water and a salt.
