Determining Conjugate Acid

Whenever we need to determine the conjugate acid of a base, do you just add one H+?

Yeah, this is correct. The conjugate acids accept an H+ from the conjugate base.
The same can be done for the conjugate base, which loses an H+.

Conjugate acids and bases deal with the Bronsted definition of acids and bases so it is proton (H+) transfer

We just add an H+, and for determining the conjugate base, just subtract an H+.

Conjugate acids gains an H+ while the conjugate bases donate an H+

You add H+ to the acid and subtract it from the base.
