Whatever your acid is in the reactant side, it would become your conj. base in the product side. Let's say your acid is HBr, after it gives of the H+, Br- is your conjugate base
And of course, vice versa with base and conj. acid

To find the conjugate acid you just introduce a proton [H+] to a base and It becomes the conjugate acid of a base. And for a base, you remove a proton [H+] for an acid to form the conjugate base. That is why they are called conjugate acid/base pairs.

When I look at the formula, I look at the reactant and it's equivalent product. If the product looks like it lost a H+, then I know that the product is a conjugate base, and the reactant for it was an acid. The same thing goes the other way: If the product looks like it gained a H+, then I know that the product is a conjugate acid, and the reactant for it was an base.

Also, the conjugate acid of a base is the molecule formed when the base accepts the proton.

the base ends up being the conjugate acid and the acid ends up being the conjugate base. The conj. base is the one where H+ gets removed and the conj. acid is the one where the H+ is added to H20.

The conjugate base is what is left of an acid when an H+ is removed, and the conjugate acid is what becomes of a base when an H+ is added.

the conjugate base is the product you have after your acid gives off a proton; the conjugate acid is what product you have after your base accepts a proton.

You find a conjugate base/acid by donating/accepting the acidic hydrogen proton in the reaction.
