Is the concentration of a conjugate acid always equal to the concentration of hydroxide ions? Similarly is the concentration of the conjugate base of a reaction equal to the concentration of hydronium ions?

This will always be true when the stoichiometric coefficients in the balanced chemical equation are the same, which is often the case when an acid or base reacts with water. The only way to make sure that concentration of a conjugate acid always equal to the concentration of hydroxide ions or the concentration of the conjugate base is equal to the concentration of hydronium ions is by ensuring that there is a 1:1 ratio of stoichiometric coefficients.
