given the ratio of concentrations how do we know that the hydrogen bromide salt remains pronated?

Because the ratio is really small, we know that the denominator is going to be a higher number. The denominator in the ratio is the protonated drug. Thus the protonated drug would be in larger quantity allowing us to conclude the drug would remain protanated. Alernatively, you can compare the pKa to the surrounding pH. Since the pKa is larger than the pH, the drug is not acidic enough in a pH of 1.7 environment that it will give up a proton. Therefore the drug will remain protanated.
