Q is the energy under any conditions, while delta H is referred to as enthalpy, and is specifically under constant pressure.

Delta H refers to the enthalpy, or the total heat/energy in a thermodynamic system; whereas q is the amount of heat/energy transferred.

Q is the energy transfer due to thermal reactions or anywhere where there is a heat transfer. Q = "energy in transit." Delta H, or enthalpy, is the state of the system or the total heat content.

delta h refers to the specific enthalpy change under a specific condition whereas q is often used as a placeholder to dictate how much raw amount of heat was released/absorbed by a thermodynamic system.

Delta H refers to Enthalpy which is the total heat in a thermodynamic system while Q is the amount of heat transferred.

Q is the amount of heat/energy transferred at any moment while H represents enthalpy, the total energy of the system.

Delta H indicates the enthalpy of the system, representing how much energy the system contains in total. On the other hand, Q indicates how much energy is transferred. I think the energy of both are usually in the form of heat for a thermodynamic system. Hope this helps!
