Final 2013 Q5 - Transition States

Can anyone please explain to me how one would come up with the transition state as shown in Q5 Part C of the 2013 Final? Which bonds are being formed/broken, and what are the two H's?

The transition state occurs when the electrons from the NH- are being transferred to the C with the attached OH but before the OH has been removed. The two H's are attached to the same C but aren't show initially because they are implied. They are only shown so that the molecular geometry can be more easily named.
