I was wondering whether Cis was with both being the same orientation or the same direction. Can it still be Cis with a an equatorial up and equatorial down or would that be considered Trans? Thanks!

that would be considered trans. In order for it to be Cis the direction must be the same; whether or not they are both equatorial or axial is unrelated.
