What factors determine how strong or weak an acid/base is?

The difference between strong and weak acids/bases is dependent on their dissociation of either hydrogen or hydroxide ions, respectively. Strong acids typically have anions with a high degree of electronegativity, allowing for their hydrogen ions to be loosely bound and prone to disassociate. Strong bases conversely have cations with a low degree of electronegativity, allowing for their hydroxide ions to more freely disassociate.

Strength is determined by the how readily acid/base dissociates into H+ and OH-.

The easier it is to dissociate into H+ and OH-, the weaker the acid or base is.
