When we know that q=-q in heat transfer, does this mean it has reached equilibrium?

no heat is transferred between two objects in thermal equilibrium

Heat flows from hot to cold and because the temperature of the two are now the same and will remain constant, heat will not be transferred between them

no it simply means heat of the system has been released to its surroundings and should equal one another.

This equation is just saying that the heat that is released by the system is equal to the negative amount of heat absorbed by the surroundings.
