I'm confused as to what you insert for qrev in delta S? Is it -delta H or nCdeltaT? How do you know when to use which?

Usually qrev will be given as a value in J transferred or absorbed as heat.

Just to add on to what Shawn said, q is the energy transferred as heat, and the subscript rev indicates that the energy must be transferred reversibly.
